Here are instructions to help with PCP lookup. Hint: a good place to start with PCP lookup is Blaze!

 "Where in the known ocular spectrum can we check to see if a doctor accepts a plan?"

  1. Blaze

  2. Carrier websites, listed on the PCP Lookup Page (Yup, you are on it rn!)

  3. Plates

  4. Provider websites (Most doctors will have a website, and some of them even list the insurance plans they accept!)

 

Humana - Select Medicare or Medicaid - Careplus

Anthem - Must select guest to find doctors.

Aetna - Must specify zip code and plan.

WellCare - Specify plan, and Medicare.

HealthNet - Centene/Wellcare

UHC JARVIS - Select zip code- UHC

Allina

Amerigroup

Empire

AllWell
