  2. login with your Assurance email for username

  3. Your password should have been set up when you became certified with UHC.

  4. If you forgot your username or password, click the corresponding ‘forgot’ links.

  5. Click the “Enrollment” dropdown

  6. Click “Medicare & Medicaid Eligibility Lookup”

  7. Toggle to “Medicaid Eligiblity” at the top

  8. Select Zip Code (This should auto-populate the state and county, sometimes a county must still be selected)

  9. Select Gender

  10. Enter Date of Birth

  11. Enter First and Last name

  12. Entering the Medicaid Number of SSN should cause the blue “Check Medicaid Eligibility” button to become clickable.

  13. An alternate option to step 12, which is usually better, is to ask for the Medicare number and click “Yes” (because shoppers are usually not as hesitant to provide this number).

  14. Click “Check Medicaid Eligibility”




  15. On the next page, toggle to “Medicaid EligiblityEligibility.

  16. This screen should provide the Medicaid number, the LIS level, and the Medicaid level. In this case, the customer qualifies for level 1 LIS and QMB Medicaid Level.
