Companionship Vendor = Papa Pals
Main Benefit Features:
Members will be matched with Papa Pals to provide a variety of companionship and general assistance needs for up to 60 hours per year.
Once a referral is made to Papa, a pal can typically get to the member’s home within a few hours and always within 24 hours.
The services Papa Pals provide include but are not limited to:
General companionship
Transportation
House help
Technology education and support
Physical activities
Grocery drop
Care gap reminders
Papa Pals will provide questionnaires such as the CDC Healthy Days, UCLA 3 Questions Loneliness Scale, fall risk assessment, and health risk assessment to assess for needs.
Papa Care Team to work closely with eternalHealth Care Management team.
Papa Pals will provide reminders to assist with care gap closures.
Members will have access to Papa Pals online platform and to phone customer service representatives.
Accessing the Benefit:
Papa’s Care Team will make outbound calls to members to enroll them in services.
Papa will match members with an appropriate Pal and will coordinate all scheduling and communication.
