Anthem MProducer

This tool will be able to help you on how to look up someone's Medicaid eligibility and get the verification code for Anthem DSNP enrollments.

Step 1: Click here to open the mproducer log in page

Step 2: Click on the orange log in button that will take you to the Usage Agreement screen.

Step 3: Click on the orange continue button the get to the home screen.

Set 4: Click on the Eligibility check Medicare and Medicaid box.

Step 5: Make sure you are on the “DSNP Eligibility Check” option.

Step 6: Enter in the following information

First Name
Last Name
State
Date of Birth
Proposed Effective Date (First of the following month)
Medicaid ID or Social Security Number
Medicare Number

Step 7: Click orange “Check Eligibility” button

Step 8: You will get a pop up with the “DSNP verification code” that you will enter into the application form. It will say not eligible if they are not eligible for the DSNP plans for that area.
