  1. Right-click this link and open in a new tab.

  2. Enter username and password which should have been received upon certification of Humana.

  3. If you forgot your username or password, select “forgot username” or “forgot password”

  4. Select Sign in

     

  5. Select “DESNP Verification”

     

  6. On the Dual Eligibility Verification page, enter the Plan Year, State, First Name, Last Name, DOB, Gender, SSN or Medicaid ID, and Medicare ID.

     

  7. The next section should state whether the customer is eligible, their Medicaid number, and their Medicaid Eligibility Level. In this case, the customer’s Eligibility Level is QMB.
