Accessing the Benefit:

• Members must contact Amplifon to make appointment.

• Amplifon will verify member eligibility and will explain details of the program over the phone.

• Amplifon will help the member locate a participating provider based on member preference and location.

• Amplifon will connect the member to the provider’s office so member can schedule the appointment directly with the provider.
